On Windows, it's PuTTY, without a doubt.  It'd be cool even if you just
used PuTTYtel (PuTTY, less ssh, for people that live in countries that
restrict crypto).  One thing I really like about it is that it mimics the
xterm copy-and-paste behavior; just highlight some text with the left
mouse button, and it gets copied to the Windows clipboard.  Right-click,
and the contents of the clipboard are pasted into your session.

As previously pointed out, pscp is PuTTY's companion scp client.  If you
want something more graphical, check out the newest addition to Rick's
list, WinSCP.  Its license allows for gratis usage.  (PuTTY's, I believe,
is free but not copyleft, more similar to a BSD- or MIT-style license.)

  Tera Term does this as well.  It's a wonderful feature - the only
drawback is that it always makes me expect my other Windows apps to
do the same...

  I'll throw in my vote for Tera Term.  I've tried SecureCRT, and it's
nice but I prefer something a little more minimalist (though not as
minimalist as PuTTY).

I second the recommendation of putty. I keep putty.exe and pscp.exe on a
floppy in my car for the rare occasions that I am away from a system with
a real OS, and out of Ricochet range. 

The putty floppy is definately a part of my earthquake emergency kit.
